To ensure we are following our mission, we screen new residents carefully. When admitting a resident, we examine the elder’s condition and any barriers to providing excellent care. We know this is teamwork and a good fit will benefit everybody. We can only win together!
Step-by-Step Admission Process:
- Resident/family communicates with us via phone/video call – takes a tour (video call or in person)
- Administrator visits your elder to meet and assess them. The meeting serves as an opportunity for your loved one to become fully aware of their move and become acquainted with the place and the administrator. We also use this time to actively engage with the elder as much as possible, to get to know them, and to answer questions they may have about our home. By the end of the meeting, the elder is usually aware of the move and knows a bit about our home. Ultimately, we want this to be their decision.
- The administrator will work with you to complete the state’s form for assessments (LIC-603 Appraisal). Family arranges paperwork required by the state. Primarily includes the Doctor completing the Physicians Report and a Tuberculosis test (LIC-602A Physician’s Report).
- Family provides information about them and their loved one (name, ID, insurance, etc.) to the administrator to help prepare admission documentation.
- Family meets the administrator and signs the Agreement.
- Family preps the room for the elder, if needed (decoration, pictures).
- Resident moves in.
- Administrator works diligently with the family for 30 days to ensure the resident is well settled.
